The availability of electronic documentation in the Sunrise Clinical Manager (SCM) system for physicians is growing. Electronic documentation allows physicians to document directly into the electronic health record and becomes immediately available for the other members of the patient’s care team.
Exercise is Medicine® (EIM) is a global health initiative managed by the American College of Sports Medicine (ACSM) that is focused on encouraging primary care physicians and other health care providers to include physical activity when designing treatment plans for patients and referring their patients to EIM program facilities.
The DeKalb Medical Wellness Center is a certified Medical Fitness facility able to provide the Exercise is Medicine program. The Wellness Center can provide a prescription pad to any doctor’s office so they are able to prescribe exercise to their patient. The patient will bring the prescription to the Wellness Center and the patient will be set up for an assessment with a wellness coach.
This program allows a continuum of communication with the physician, the patient and the Wellness Center providing the best opportunity for success for the patient.
